**The Football/Cheer Health Form 2026 is NOT able to be accessed with the rest of the forms on RecDesk. This form will be sent to you via email and MUST be completed, signed by a doctor, and returned by the start of practices.

WHAT TO WEAR/BRING
Participants should come wearing comfortable clothing (sweatpants/shorts and t-shirt) and athletic shoes (sneakers preferred, cleats NOT allowed). The Field of Dreams is an outdoor complex, so please dress accordingly for the weather. Layers are recommended (sweatshirts, jackets). Participants must bring their own water bottles as these are not provided during the program.

WHERE TO WATCH
Parents/guardians are required to participate alongside their child if needed. If your child does not need 1:1 support and you prefer not to participate as well, please watch from the sidelines of the field. Visiting children/siblings/family members are welcome to watch as well. Parents and family members are welcome to bring chairs and/or blankets; there is limited seating along the sidelines.

PHOTOS
You are welcome to take pictures of your child during the program. Please be mindful to take photos of your child only and ask permission to take photos of other participants.
