(The AC Screening Form allows our staff to have a better understanding of your child and their individual needs prior to the start of the program. The Health Form contains information about seizures, allergies, and asthma, informing staff if your child has any additional needs they should keep an eye out for. Activity Connection forms are annual, so you need only complete them once a year.)


WHAT TO WEAR/BRING
Participants should come wearing comfortable clothing (sweatpants/shorts and t-shirt) and athletic shoes (sneakers preferred, cleats NOT allowed). The Field of Dreams is an outdoor complex, so please dress accordingly for the weather. Layers are recommended (sweatshirts, jackets). Participants must bring their own water bottles as these are not provided during the program. Baseball mitts are recommended but not required. Metal/wooden bats are NOT allowed; all participants will be using provided plastic bats for safety purposes.

WHERE TO WATCH
Parents/guardians are required to participate alongside their child if needed. If your child does not need 1:1 support and you prefer not to participate as well, please watch from the sidelines of the field. Visiting children/siblings/family members are welcome to watch as well. Parents and family members are welcome to bring chairs and/or blankets; there is limited seating along the sidelines.

PHOTOS
You are welcome to take pictures of your child during the program. Please be mindful to take photos of your child only and ask permission to take photos of other participants.
