REQUIRED FORMS
All paperwork is due no later than May 31, 2026. Any outstanding balances after this date will result in your child being removed from the roster in order to accommodate our long waitlists.

DURING REGISTRATION on RecDesk please submit the following:
1. Camp Pals Screening Form
2. Approved Pick-Up List
3. Media Release Form
4. Health Form

A non-refundable camp deposit of $75 must be paid either online or via check at time of registration. Full camp registration fee of $450 must be paid by check or online before May 31st, 2026.

VIA EMAIL or MAIL please submit before May 31, 2026.
1. Medical Packet signed and stamped by primary care physician
a. Health Form
b. Medication Form – All medications must be submitted to Camp Nurse at check-in (if applicable). Camp Nurse will NOT accept any over the counter medications; only medications with doctor's signature and prescribing order will be accepted.
c. A copy of immunization records (collect from physician and submit with medical packet)

PAYMENT
Payment for all campers is due before May 31, 2026.
Any financial assistance documents must be submitted as soon as possible and approval must be communicated to Activity Connection staff no later than June 1, 2026. Our top priority is ensuring all teenagers and young adults have access to our programs. If there are concerns with registration fees, please contact our staff as soon as possible to discuss some of our many financial assistance options.

If paying via Check:
Check must be made payable to: CHILDREN’S SPECIALIZED HOSPITAL with “Camp Pals Paradise” written in the Memo of the check

Camper registration ends April 30, 2026.

*If requested, you may receive a partial refund before June 1, 2026. No refunds will be granted after this date. The deposit is non-refundable.*
