Note:

DURING REGISTRATION on RecDesk as “Camp Chatterbox-Family Member Under 18,” please submit the following online:
1. Media Release Form
2. Statement of Agreement
3. Health Form
4. Approved Pick Up List (one per child under 18)
5. Non-refundable camp deposit of $150 paid either online or via check; full camp registration fee of $500 must be paid by check or online before May 31st.

VIA EMAIL, FAX or MAIL please submit before May 31st, 2026:
1. Medication Form (if applicable)
2. A copy of immunization records (collect from physician and submit via email)

PAYMENT
Payment for all attending family members is due before May 31st, 2026. Any outstanding balances after this date will result in your child being removed from the roster in order to accommodate our long waitlists. Our top priority is ensuring all teenagers and young adults have access to our programs. If there are concerns with registration fees, please contact our staff as soon as possible to discuss some of our many financial assistance options.

If paying via Check:
Check must be made payable to: CHILDREN’S SPECIALIZED HOSPITAL with “Camp Chatterbox” written in the Memo of the check.

Family registration ends on April 30, 2026. No family members will be able to register after this date, no exceptions.

*If requested, you may receive a partial refund before June 1, 2026. No refunds will be granted after this date. Deposit is non-refundable.*
