Note:

The following forms will be required for participation. Most forms can be completed online through your RecDesk Profile, with the exception of the Health Form, as it needs a doctors signature. This form will be sent via email.

Completing AC Forms allows Field of Dreams staff to have a better understanding of your child and their individual needs prior to the start of the program. The Health Form contains information about seizures, allergies, and asthma, informing staff if your child has any additional needs they should keep an eye out for.

AC Forms are annual, so you need only complete them once a year.
