Note:

DURING REGISTRATION on RecDesk as “Camp Chatterbox-Camper,” please submit the following:
1. Approved Pick-Up List (1 per camper/sibling under 18)
2. Media Release Form
3. Communication Device Form
4. Activity Connection Screening Form
5. Statement of Agreement
6. Health Form
7. Non-refundable camp deposit of $200 paid either online or via check; full camp registration fee of $1,000 must be paid by check or online before May 31st, 2025.

VIA EMAIL, FAX or MAIL please submit before May 31, 2025.
1. Medication Form – All medications must be submitted to nurse at check-in (if applicable)
2. A copy of immunization records (collect from physician and submit with medical packet)
3.A copy of current Speech, OT, and PT reports.
4. A copy/picture of device screens or overlay
5.. A recent photo of camper.

PAYMENT
Payment for all attending family members is due before May 31st, 2025.

If paying via Check:
Check must be made payable to: CHILDREN’S SPECIALIZED HOSPITAL with “Camp Chatterbox” written in the Memo of the check.

Camper registration ends April 30th, 2025.

*If requested, you may receive a partial refund before June 1, 2025. No refunds will be granted after this date. Deposit is non-refundable.*
