Camp Chatterbox-Camper

Camper Criteria:
1. Children and youth 5-22 years old who communicate using dynamic voice output AAC systems.
2. Campers must be able to participate in therapy and recreation activities without requiring one-to-one behavioral support.

Upon registration, campers must complete Activity Connection Screening Form (online).

To complete registration the following documents must be completed and submitted prior to April 1st, 2020 (forms, other than immunization records, will be provided via email from Camp Chatterbox Coordinator):
1. Activity Connection Medical Packet (includes health form and emergency action forms, if applicable, signed by physician)
2. Detailed list of medications
3. Immunization Records (retrieve from your primary physician)

Full camp registration fee must be paid within 4 weeks of acceptance.
Deposit will be returned if child is not accepted into the program.

Camp Chatterbox Statement of Agreement
1. Camp Chatterbox is responsible for any loss or breakage of any of the LEASED property, as well as additional costs incurred by our stay. Therefore, all families agree to respect camp property and to help keep the facilities in the same condition as they were found upon our arrival.
2. All children, both campers and siblings, must be in a structured camp activity or under a parent’s supervision at all times. No child is allowed to roam outdoors or in any building without a parent or staff member present.
3. At least one parent from each family must remain on the camp premises at all times unless permission to leave is given by the Camp Chatterbox Directors.
4. Siblings must be mature enough to participate in camp activities without one-on-one supervision. Siblings are expected to be positive and respectful of all campers.
5. Prior to the start of the camp program, families must obtain permission from the Director of Camp Chatterbox to have any visitors at camp. Camp staff is to be notified of all visitors immediately upon arrival.
6. Camp therapy staff and counselors will exercise reasonable care in working with campers and their AAC and/or mobility equipment. Parents accept financial responsibility for any device repairs or technical problems that occur during the camp program.
7. No parents, siblings, nor campers are permitted to use the computers of the Leasing Agency, the Camp Office and/or its equipment.
8. Each family within the Camp Chatterbox group is responsible for cleaning their cabin prior to departure and leaving it in the same condition as it was upon arrival. All cabins will be inspected by the Camp Chatterbox Director or designee prior to each family’s departure.
9. Upon arrival and departure families may drive onto the campgrounds to unload their vehicles and then all vehicles are to be parked in the designated parking area during camp.
10. Smoking is not allowed in any camp buildings. Please smoke only in designated areas and dispose of all cigarette butts properly. Cigarette remains are not to be left on the ground as this is considering littering.
11. Personal pets are not permitted on the campground at any time.
12. Shoes must be worn at all times, except in the pool area as directed.
13. Campers, parents, siblings and staff agree to abide by the recreational and water safety regulations of the camp facility.
14. Parents, siblings and campers are prohibited from bringing the following onto camp property:
a. Illegal drugs
b. Guns or anything that can be considered a weapon
c. Alcoholic beverages
d. Flammable materials

I have read the above stated Camp Chatterbox regulations and agree to ensure that I, along with all members of my family that attend Camp Chatterbox, will abide by them. I understand that if I, or any member of my family, violates any terms of this agreement, our family will be asked to leave camp immediately. I understand that we will not be eligible for any refunds nor to return for future Camp Chatterbox programs or activities.
